Susquehanna Valley Futsal is a league in the USYF, United States Youth Futsal.
USYF is an affiliate of USSSA, United States Specialty Sports Association.
USSSA provides insurance for players, coaches and officials in the SVFutsal League.
SVFutsal requires all officials to be insured before you can officiate games.
For 18 and older, SVFutsal will pay $30 of the $65 insurance expense.
For 17 and younger, SVFutsal will pay $15 of the $27 insurance expense.
If an official has a USSF Grade 8 or Grade 9 Futsal Referee Certification, they are insured under their USSF policy.

You will sign up as an all sport official. Your primary sport is soccer. After payment be sure to print out, or save a copy of your payment confirmation.
USSA Official's Insurance Information (Coverage Details)
Payment for the upcoming year insurance policy, is usually available in December.
DO NOT PAY for insurance until you receive an official announcement or until Jan 1 or later of the year you want your policy coverage.
If you want to get re-imbursed by SVFutsal, you must provide a copy of the USSSA Payment Confirmation to the league director.
Ensure the home postal address on the payment confirmation is correct
Paper copies are acceptable, but NOT PREFERRED. Instead of paper, upload a copy of your confirmation to the referee web site.
Use the following format to rename your PDF payment confirmation file.
YEAR USSSA Insurance Confirmation Lastname Firstname.PDF
